Children/Youth Leader Background Check
Use this form to request a free background check on anyone wishing to volunteer with children and/or youth. Many insurance carriers now require background checks on anyone who works with children/youth. The form is also available by calling the Pretrial Services department of the Administrative Office of the Courts at (800) 928-6381.
